Required Forms for Participation

We will need the following forms before your child will be eligible to participate in the camps, clinics, or on the first day of practice. The Sierra Athletic Conference (SAC) Medical Clearance form must be printed and filled out by a licensed physician and dated after April 15, 2023, but before participation in any camp or clinic activities.
 

• CCS Athletic Participant Application, Waiver, and Release
• SAC Medical Clearance Form date after April 15, 2023
• Recent Report Card or School Enrollment Form 
• Copy of Certified Birth Certification (New student athletes only)
• Student Athlete/Parent Code of Conduct
• Social Media and Media Release Policy
• Heads Up Concussion Form
• Camp(s)/clinic(s) forms*
